My question is: Can a loaned amount be forgiven and adjusted as Zakat? If the original intention when giving the loan was to have it repaid, but later, realizing the person is deserving (mustahiq), one wishes to forgive it with the intention of Zakat?

Wonderful, MashaAllah! You gave a loan, and when you realized the person was deserving, you forgave it with the intention of Zakat. Allah Almighty will reward you for both—giving the loan and giving Zakat. There is actually more reward in giving a loan than in giving charity (Sadqah). I find it easy to give charity, but I am unable to give loans. MashaAllah, go ahead and do so.
