You can specify things like where you will or won't go, or that you require a certain amount of income/allowance, etc. You can definitely do that; obviously, the marriage will only happen if the man agrees to those terms.
As for the condition restricting the husband from a second marriage: no, you cannot strictly forbid what Allah has allowed. Even if he marries again without the first wife’s consent, it is Halal (permissible). In such a case, you simply have the option to step back and leave the marriage if you choose. All other conditions are valid, but not the one banning a second marriage entirely.
Another important point is the right to divorce (Tafweez-e-Talaq) for the woman. In my view, this is often unnecessary because women already have the right to Khula (divorce initiated by the wife), which serves the same purpose as the right to divorce.
